Payroll Setup Forms

Thank you and welcome to Custom Payroll Associates, Inc. You may download the forms from the link below to begin setting up your payroll account. Please feel free to call us at 972-353-0000 to go through the forms with you, answer any questions and finalize your payroll setup. This Payroll Setup Forms Kit is for companies that currently have an existing payroll or for companies that do not have any payroll this calender year.

To setup your company for payroll processing services we will need:

• Copy of a company check on the account from which we will draft payroll. (May be voided)
• CPAI New Client Information Sheet – 2012
• New POA-2848-2012-1 (State & Local POA's if required)
• CPAI-Service-Agreement, signed
• EMPLOYEE PROFILE 2012
• Copies of Current Yr. 941’s, Copies of State and local returns (if applicable)
• Copy of IRS document with Company name and Company EIN, for verification
